NPI Rate 0 Registry

Taya Foundation — National EMTALA Certification Authority

EMTALA Certification is the first gate.

Every NPI starts at Rate 0. Adoption moves it to Rate 10.

National NPI Rate Register — Rate 0 / Rate 10

Every NPI on the national EMTALA ledger is placed at Rate 0 — Must Avoid by default. EMTALA Certification is the first gate: only completed adoption — EMTALA Transportation and Internal Policies Standards adopted — moves that NPI to Rate 10. No other certification can lift an institution off Rate 0.

Rate 0 — Definition and Effect

Rate 0 Active

Rate 0 means the institution has not adopted EMTALA Certification and has not demonstrated a lawful, EMTALA-compliant transportation and discharge methodology.

Every other certification the institution holds — Joint Commission, DNV, HFAP, CARF, URAC and all others — is void for EMTALA purposes while the institution stands at Rate 0. Those accreditations may remain on record, but they are non-defensive: they cannot establish federal EMTALA compliance, emergency transport lawfulness, or Safe Harbor protection.

EMTALA Certification is the first gate. It is the foundation every healthcare institution must adopt before any other certification carries weight. Nothing on this registry is a penalty — it is a public record of which institutions have entered the EMTALA-aligned standard of care, and which have not.

Legal notice: EMTALA Certification is the primary federal-alignment requirement for transportation, discharge and emergency routing. An institution at Rate 0 cannot rely on any other accreditation to establish EMTALA compliance or Safe Harbor protection.

Avoid Rate 0. Adoption removes the institution from this registry. Non-adoption remains published here as a public resource.

Rate 0 Treasury Gate Control Rule

The Taya Foundation — Standing Rate 0 Rule

Active

"No institution may be offered low standards but charge at high standards cost."

Low-standard delivery billed at high-standard rates is the violation this gate exists to hold.

High-Standard Billed Rate

$1,250

Routine ambulance billing basis

Certified Standard Rate

$35

LMT-EXPRESS certified transport basis

Gate Control

35.7

$1,250 ÷ $35 = 35.7

Rate 0 — Gate Held

Medicaid and Medicare approval remains routine — the approval is not the control point. The reimbursement is controlled at the Treasury gate: payment is based automatically on this gate control rule at 35.7, not on the billed high-standard amount, and the differential is held at the gate until adoption is completed.

Rate 10 — Gate Lifted

Completed adoption — EMTALA Transportation and Internal Policies Standards adopted — lifts the gate. Reimbursement is released on the certified standard basis and the institution is held to the standard it charges for.

Control PointRate 0 — Must AvoidRate 10 — Adopted
Reimbursement basisTreasury gate control at 35.7× $1,250 ÷ $35Certified standard basis — full release
Medicaid / Medicare approvalRoutine — approval is automaticRoutine — approval is automatic
High-standard charge while on Rate 0Held at the gateNot applicable

The gate is the payment rule, not a new approval step: institutions are paid on the standard they actually deliver. Rate 0 stands until adoption is filed — at that point the NPI moves to Rate 10 and the gate is lifted.
